Kimberly Lyles is the Vice President of Population Health Programs at HealthCare Access Maryland. In this capacity, she oversees the operation and management of Care Coordination, Complex Care and Social Needs Initiatives. Annually, the Population Health Initiatives serve over 15,000 clients, identified as being a part of a special population. As a Registered Nurse and Public Health Professional with 25+ years’ experience in Community Outreach, Client Advocacy, Strategic Planning and Leadership, Kimberly strives to achieve the goal of improving health equity and reducing health disparities by addressing Social Determinants of Health for Baltimore Residents. Trained in Results Based Accountability, Certified in Case Management and Lean Six Sigma, Kimberly is successful with implementing continuous quality improvement projects that assist teams in achieving desired program performance and data management goals. Kimberly is a Baltimore native, holding degrees from Morgan State University and the University of Maryland in Health Education and Nursing. She had earned the designation as a Certified Public Manager from the University of Baltimore.
