The Complex Care and Social Needs Department works with individuals who need assistance with behavioral health and social need challenges. We provide care coordination and navigation services, partnering with community settings such as: Emergency Departments, Prisons and Jails, and Recovery Treatment programs to address identified social determinants of health clients need to ultimately empower them for a positive outcome.
