HealthCare Access Maryland is launching a new Community Diabetes Initiative. Join us Wednesday, January 24th from 1-2pm for a soft launch event on the Mezzanine level of HCAM’s offices. Meet our team of Community Health Workers and learn how we, along with our community partners, will be making Maryland healthier through diabetes education, community outreach and engagement, and connection to local Diabetes Prevention Programs (DPP).

Our Community Health Workers will be conducting Pre-Diabetes Risk Assessments and providing referrals to Diabetes Prevention Programs.

This new initiative is a partnership with the Baltimore City Health Department.
